Your face after weight loss: what massage can and cannot do

GLP-1 medicines and fast weight loss change the face. What the research measured, what lymphatic and buccal massage can help with, and what they cannot replace.

Many people losing weight this year, often on a GLP-1 medicine such as Ozempic, Wegovy or Mounjaro, feel better and find their face looks older than they expected. The internet calls it "Ozempic face." It is not a diagnosis. It is what fast weight loss does to the face, whatever caused it.

Massage can help with some of these changes but not all of them. Below is what I tell clients at consultation.

What changes, and what was measured

The face has fat pads, some just under the skin and some deeper, against the bone. They give the cheeks and temples their fullness. When the body loses fat, the face loses some too.

The most useful imaging study so far is small. Surgeons in the United States measured the midface on CT and MRI scans of 20 people before and after GLP-1 treatment (Otolaryngology–Head and Neck Surgery, 2025). Midface volume fell by a median of 9%, about 7% for every 10 kilograms lost. The fat just under the skin lost more than the deep fat. The scans were taken for other reasons, there was no comparison group, and nobody was followed afterward, so the study cannot say whether the medicine itself does anything beyond the weight loss.

Facial plastic surgeons describe the same picture: less fullness, more visible lines, and looser skin (Facial Plastic Surgery, 2023). The skin that stretched over the old volume does not shrink back at once, especially after forty.

What massage can help with

  • Puffiness. Weight loss does not stop fluid from sitting under the eyes and along the jaw. The European Lymphatic Facial Massage moves it out within hours, and a face that is hollow in the cheeks and puffy under the eyes looks more tired than either alone.
  • Tension. Many clients clench, and the masseter tightens and pulls on the lower face. The Buccal Lift releases the chewing muscles from inside the mouth, so the lower face sits higher and softer.
  • Circulation. Massage brings blood to the skin. A small study at the Tokyo Institute of Technology found five minutes of roller massage raised blood flow in the cheek by up to about 25%, and the rise lasted more than ten minutes (Complementary Therapies in Medicine, 2018).
  • Tone. Muscle is part of the shape of the face. In a Northwestern study, 16 women who did thirty minutes of facial exercises a day for twenty weeks showed fuller upper and lower cheeks in photographs rated by dermatologists (JAMA Dermatology, 2018). That was exercise, not massage, and a small group. It still tells me the muscles under a thinner face are worth keeping in use.

What massage cannot do

Massage does not replace fat. It does not rebuild collagen on its own, and it will not tighten skin that has stretched. Volume is medical work: fillers, fat grafting or surgery, decided with a board-certified dermatologist or facial plastic surgeon. I am a Texas-licensed esthetician, and those are not my tools.

What I can do is keep the face you have decongested, released and cared for while the rest of your body changes, and tell you honestly what I see at each visit. I explain the limits of massage before a client books a series.

If you are losing weight now

  • Tell your prescriber how your face is changing. The pace of loss is their decision, not mine.
  • Keep your skin barrier strong and wear sunscreen every day. Sun damage shows faster on thinner skin.
  • Book a consultation before you book a series. I will look at your face, ask about your medicine and your timeline, and tell you whether lymphatic, buccal or The Zhanna Method fits, or whether you need a doctor first.

The studio is in Sawyer Yards, and I see three clients a day.

Frequently Asked

The questions clients actually ask.

What is Ozempic face?
It is the popular name for the facial changes that come with fast weight loss, on GLP-1 medicines or otherwise: less fullness in the cheeks and temples, more visible lines and looser skin. It is not a medical diagnosis.
Can facial massage fix Ozempic face?
Not on its own. Massage can take down puffiness, release tight jaw muscles and bring blood to the skin, which helps the face look rested. It cannot replace lost fat or tighten stretched skin. That is medical work for a dermatologist or facial plastic surgeon.
Is lymphatic drainage safe while taking a GLP-1 medicine?
For most people, yes, but Vera Pelle reviews a health intake before every first session. List the medicine and any recent changes in dose, and tell your prescriber you plan to book.
